Illinois HIPAA Health Insurance Plans - What You Need to Know About the IL HIPAA Regulations

The Illinois HIPAA health insurance laws are designed to protect the health care rights of all Illinois residents and prevent “job lock”. If one is eligible for an IL HIPAA health insurance plan then the plan will be issued on a guaranteed issue basis and all pre-existing conditions will be covered immediately from day one.

The state of Illinois is one of the most important ones in the whole continental United States. It is home to 12,831,970 people and its population has increased by 3.3% since the year 2000. All this changes have led to regulation from the Federal Government, specifically Health care regulations. The state of Illinois like many others, offers low cost health insurance for many people, however there are many more that cannot be covered because they are non eligible or they simply think that the type of state funded care is not comparable to one that they can buy by them.

In August 26, 1996 the Federal government passed the Health Insurance Portability and Accountability Act (HIPAA) that changed the health care insurance company world drastically. The act calls for insurance companies to improve efficiency in the health care system delivery by standardizing electronic data interchange and to protect confidentiality and security of health care data through setting and enforcing standards.

While one of the most important aspects of the HIPAA laws to remember if you are an Illinois resident looking for health insurance (or really a resident of any of the 50 states since HIPAA is a Federal law) is that HIPAA allows for guaranteed issue coverage if one meets certain criteria. Speak with a qualified health insurance agent or financial planner for more details on the specific HIPAA requirements.

Complying with all requirements of this law will require providers and all entities within the health care industry to make significant changes to their computer information systems, business practices and procedures. If companies did not comply with the regulations they would be subject to heavy fines up to $25,000 a year! The deadlines for this law were divided into the electronic standards (October 26, 2002) and the privacy standards (April 14, 2003).

Within the state of Illinois the same provisions under federal law apply. The state of Illinois published new rules that would ensure standardization of electronic patient health, administrative and financial data, and unique health identifiers for individuals, employers, health plans and health care providers, and protecting security standards for individual’s intimate information. The HIPAA’s regulations can be viewed as an “administrative simplification” which is composed of four parts.

Standard for Electronic Transactions: This section out of the four is the one that mandates for computerizing the health care industry. Prior to this they would find many ways to implement and manage all their customer information. It includes health plan claims, health plan eligibility, enrollment and disenrollment, payments for premiums and claim status. An example of the electronic standard transaction would be the health care companies adopting codes such as coding systems that describe injuries and types of plans.

Unique identifiers for employers, providers and health plans: This part of the plan just makes sure they providers and employers can exchange private and valuable information with each other error free. They require employers to adopt an employer’s tax ID, or employer identification numbers. Although the plan calls for this, a final Health Plan identifier has not yet been finalized.

Security Rule: This third section of the plan provides a uniformed level of protection for information about any individual that is private and transmitted electronically. It requires entities and health care providers to ensure the confidentiality and availability of an individual’s information and to protect against any hazards or the security of such information.

Privacy Rule: The last section of the act may be the most important one for the health care industry to follow and for the customers to know. This section of the plan just makes sure that any individual’s private information is not shared unless is needed by the coverage agency, even if the information is saved in electronic format. The rule also has a basic set of national privacy standards regarding what insurance companies must do with the information and the patients. The national privacy standards include but are not limited to the following: give patients new right to access their own medical records, restrict access to medical access by other people (not the patient), provide that all patients are notified of covered entities’ privacy practices, establish criminal as well as monetary sanctions for the improper disclosure of private information and establish business associate agreements with business partners that safeguard the private information.

HIPAA regulation is one of the most important pieces of legislation regarding Health Care laws in the last decade. To the Federal Government and the state of Illinois is not only important to provide Americans with plans that can assist them with their health insurance needs, but they also want to protect Americans with their privacy and allow them to trust their health insurance entities.

Children And Birds - Things To Consider Before You Bring Your Pet Bird Home

Children and animals make great companions. Regardless of the type of pet that you own there are always certain precautions to take. If you are considering having a bird as a pet, keep the following things in mind.

The biggest challenge when combining birds and young children is how they treat the animals. If you had a bird while your child was growing up, then they should have learned the proper technique to handle and feed these wonderful creatures. However, if you bought a bird while your child was still young, they may have several things to learn.

The bottom line is that the majority of the tasks associated with owning a bird will fall on the parent and not on the children. It will be the parents’ responsibility to make sure that the few chores that the child can do are done accordingly. This will include things like cleaning the birdcage or dispensing food.

As is often the case, young children do not understand how to handle a bird and are not aware of how fragile these amazing creatures can be. A bird is a very soft creature and is not a tough toy like your child may be use to playing with. If your child is overly aggressive with other animals, you probably do not want a bird at this point as the chances of injuring the bird or making them erratic is very good.

There are often many species of birds that are suitable for a child. But some species prefer more quiet and less energy driven areas. If you bring a bird home, which is not suitable for this habitat, it could be aggressive, noisy, and even experience many health problems. .

Canaries, budgies, lovebirds and cockatiels are perfect pets for an older child. There are also many types of parakeets and some small conures that would also be good choices. Typically these creatures are very social, and should be kept in an area of the home that is usually attended by people as well. These creatures should not be kept in the kitchen however, as they can be very messy. Also a child’s bedroom is not a good place for these wonderful creatures, unless your child spends most of their time there. However, having a second birdcage in the bedroom is a good idea.

5 Diabetic Cookies and Cake Recipes

Being diabetic can be difficult, especially when it comes to eating the right foods. Here are 5 random cookie and cake recipes that a diabetic can enjoy as well as anyone else who is hungry.

DIABETIC CAKE

2 c. raisins

1 1/2 c. water

1/2 c. orange juice

1 c. unsweetened applesauce

2 eggs, beaten (or Eggbeaters)

2 tbsp. liquid sweetener

1/2 c. cooking oil

2 c. self-rising flour

1 tsp. baking soda

1 tsp. cinnamon

1/2 tsp. nutmeg

1 tsp. vanilla

Slowly boil raisins in water until water is absorbed. Add orange juice, applesauce, sweetener, eggs and oil; mix well. Blend flour, baking soda, cinnamon and nutmeg; add to first mixture together with vanilla. Mix well. Pour into greased 9-inch pan. Bake at 350 degrees for 25-30 minutes.

OATMEAL & APPLESAUCE DIABETIC COOKIES

1/2 c. flour

1 1/2 tsp. cinnamon

1/2 tsp. soda

1/4 tsp. salt

1/4 tsp. nutmeg

1 tsp. allspice

1/2 c. oatmeal

1/2 c. raisins

1/2 c. unsweetened applesauce

1/4 c. cooking oil

1 egg

1 tsp. vanilla

Mix flour, cinnamon, soda, salt, nutmeg, allspice, oatmeal and raisins. Add applesauce, egg, oil and vanilla. Mix to moisten and drop onto greased cookie sheet. Bake 12 minutes at 375 degrees.

DIABETIC DATE CAKE

1/2 c. butter

1 tbsp. liquid sweetener or 1/2 c. sugar

1 egg

1 c. finely sliced dates or raisins

1 1/2 c. diabetic applesauce

1 c. chopped pecans (optional)

1/4 tsp. cloves

1 tsp. vanilla

2 c. flour

2 tsp. soda

1/2 tsp. cinnamon

Cream butter, egg and applesauce. Add liquid sweetener and vanilla. Sift dry ingredients together; add to creamed mixture. Beat until well blended. Turn into loaf pan and bake at 350 degrees for 1 hour.

DIABETIC DATE BARS

1 c. chopped dates

1/3 c. vegetable oil

1/2 c. orange juice

1/4 tsp. artificial sweetener

1 c. flour

1 tsp. baking powder

1/2 c. chopped pecans

1/4 c. Eggbeaters (or 1 egg)

1 tbsp. grated orange rind

Boil dates, oil, and orange juice for 5 minutes and cool; add rest of ingredients. Mix all together and spread in oiled 8 x 8 inch baking dish. Bake at 350 degrees for 25 minutes. Cool before cutting. Yields 36 bars. Each bar: 56 calories. 7 grams carbohydrates, 1 gram protein, 3 grams fat, 8 mg. cholesterol, 12 mg. sodium.

DIABETIC COOKIES

1 c. all-purpose flour

1 c. quick oats

3/4 c. seedless raisins

1/2 c. orange juice

1/2 c. butter or margarine, softened

2 tsp. baking powder

1 tsp. grated orange peel

1/2 tsp. salt

1/2 tsp. cinnamon

1 egg

3/4 c. nuts

Sugar substitute to taste

Mix all dry ingredients plus oats, raisins and orange peel. Add orange juice, egg, oil and vanilla. Then add nuts. Mix to moisten and drop onto greased cookie sheet. Bake 12 minutes at 375 degrees.

Credit Repair and the End of Authorized User Accounts

Common Practice

It is a common practice for parents to give a child a credit card. It’s good for emergencies and handy for everything from summer camp to college life. It is also common for one spouse to make the other spouse an additional card member.

A Little Software Bug

None of these practices are even slightly questionable. Additional card member accounts didn’t attract the attention of lenders or Fair Isaac Corporation, the creator of the credit-scoring model used by lenders when they make credit decisions, until they noticed an interesting bug in the credit-scoring model which was to spawn a questionable little sub-business in the credit repair industry.

A Cool Credit Repair Trick

It seems that as soon as the new account appeared on the credit report of the beneficiary their credit score would jump. In other words they were inheriting the credit history of the card from the donor. In the case where the donor had excellent credit the beneficiary discovered their own credit score jumping significantly, often over one hundred points, a cool little credit repair trick, but not what Fair Isaac really had in mind.

Word Spread Quickly

Well, the word quickly spread. And as always happens, creative entrepreneurs thought up a way to package this small credit repair miracle and sell it to absolutely anyone who had the money and desire to give their credit score a quick, albeit undeserved boost. And it is this undeserved issue that became the real problem.

FICO in our Life

In 1989 Fair Isaac Corporation first made their FICO credit scores available to lenders. And in 1995 the mortgage lending giants, Fannie-Mae and Freddie-Mac made FICO scores a part of their mortgage underwriting consideration. Since that time lenders have become virtually dependant on Fair Isaac’s scoring model, and the credit repair services began to focus on these powerful little numbers.

Score Distortion

The reliability of these important numbers is essential to good decisions. If something were to threaten the validity of the FICO score undeserving borrowers would be able to obtain loans they did not deserve. Lenders making these loans would be in jeopardy of increasing default rates and potentially disastrous losses.

Credit Repair, or is it?

So you can image the concern that lenders felt as they discovered that pseudo credit repair companies were popping up on the Internet selling these accounts to anyone with the money to pay. In response, Fair Isaac went to work to modify their computer model to block the benefit of these accounts, and to end to the distortion being caused by credit repair card sellers. And so they rewrote the model and introduced it as FICO ‘07. But there was a problem.

The Credit Repair Battle Continues

As much as lenders wanted to eliminate the score distortion caused by authorized user accounts, they were unable to implement FICO ‘07. It seems that there is a paragraph in the Equal Credit Opportunity Act (ECOA) that states that, “when lenders assess a spouse’s credit risk they are required by law to consider the credit history of accounts which both spouses are permitted to use.”

Back to the Drawing Board

This little directive stopped FICO ‘07 in its tracks and sent Fair Isaac back to the drawing board. For those of us in the credit repair business, we were amused to see the fanfare amongst the brokers of these authorized user cards that trumpeted FICO ‘07 as “illegal”, and of course continued to sell their questionable credit repair product to the public. Their celebration, however, was short lived.

Credit Repair Card Brokers Say Goodbye

On July 31, 2008, Fair Isaac announced their final solution. They have managed to create a way to block questionable authorized user accounts from inheriting the score benefit of the donor without blocking those of spouses and family members. So finally the death blow was dealt against the shadowy credit repair businesses selling their quick fixes.

Those Amazing Brainiacs

The exact language of the press release reads, “We have developed technology that will reduce any impact on the FICO 08 score from intentional tampering, while allowing the scores of spouses and other genuine authorized users to benefit from their shared credit experience.” How this works, I can’t say, but my hat is off to the brainiacs at Fair Isaac.

Credit Repair the Right Way

Intentional distortion of the validity of credit scores was good for no one. Intelligent credit repair never requires questionable practices. There have always been many legal and powerful ways to boost your credit score, quickly and effectively. A well-planned approach to credit repair has always produced real and lasting results that endure and truly reflect your own credit worthiness. Do it right, and do it for real.

Top Ten Writing Mistakes Made by New Children’s Writers

Okay. So I’m not David Letterman. But I doubt if he’d know much about the top 10 mistakes made by new children’s writers anyway. I, on the other hand, read from 10 to 20 manuscripts for children every week (I’m not bragging - I’m just an instructor with the Institute of Children’s Literature). While many of the stories I read are destined for publication, I find that 10 common mistakes crop up again and again in the other manuscripts I edit each week.

I’ll start with number 10 (just like Letterman) and work my way up to the number one writing mistake made by new children’s writers (and, just so you know - I’ve been guilty of making some of these mistakes myself, so don’t beat yourself up if you realize you’re guilty of some of these, too):

10) No Clear POV Character - Children tend to relate to the POV character in a story. This is the person they will root for. Make it clear right from the start whose story is being told. Even if you have two main characters (twins, for example), you need to pick just one of these kids to be your POV character. And, it should go without saying, when writing for children, make sure your POV character IS a kid - even if Grandma has a big part in your story.

9) Multiple Points of View - Unlike stories for adults, stories for children are generally told from only one POV. It isn’t difficult to maintain a single point of view once you get the hang of it. Just remember - if you are “showing” everything from your main character’s point of view, then he or she has to be present for everything that happens. I see stories all the time where the POV character suddenly leaves the room. Yikes! If your POV character wasn’t there to see or hear what went on, then we can’t see or hear it either.

8) Telling instead of Showing - Read a good story and chances are there is a lot of action and dialogue (showing) with minimal stretches of straight narrative (telling). Too much narrative and the story sounds like a summary. Readers don’t want a summary. They want scenes with action and dialogue that make them feel they are actually experiencing what is going on. So “show” as much as possible of your story through action and dialogue.

7) Overuse of Adjectives, Adverbs, and Other Unnecessary Words -Do you really need to say someone “whispered quietly” Or “shouted loudly” Or, my favorite - she “nodded her head”? What else could she nod? Or, she “shrugged her shoulders” - she certainly wouldn’t shrug her foot!

6) Dialogue That is Not Punctuated Properly - Get a grammar book to learn how to punctuate dialogue properly. But, most importantly, remember to change paragraphs each time the speaker changes. I read manuscripts all the time where three or four characters are speaking, yet the paragraph never changes. Just imagine how confusing that is to the reader!

5) Long Timeframes - I know Harry Potter takes place over several years. But, the story also takes place through several books. Most children’s writers start out writing stories for children’s magazines or they want to write picture books for very young children. Either way, the timeframe in these stories should be rather short - a couple of hours or a day or two. If your story takes place over a couple of weeks or (gulp!) a couple of years, then you need to shorten the timeframe.

4) No Narrative “Hook” for the Reader - I know what you’re asking -”What is a narrative hook?” Well, that’s simple. It’s just an opening sentence or two that “hooks” the reader and makes him or her want to continue reading to find out what happens.

3) Dialogue That Doesn’t Sound Real - Listen to any child or teenager and you’ll find out that much of what kids and teens say (at least to each other) tends to sound like a series of grunts. So don’t have the child or teen in your story use words like “shall,” or never use contractions. If you do, the dialogue will sound too formal and your work will not have a child’s or teen’s voice.

2) Adults Who Step In to Save the Day for the Child - I know what you’re thinking. Parents and other well-meaning adults DO step in all the time to save the day for kids. So why can’t they do it in stories for children? The answer to that is - because children don’t want to read stories like that. Stories for children have strong children (or children who eventually become strong throughout the course of the story) as characters. This empowers the children who read these stories. They figure, if the POV character can solve his own problems then maybe they can too.

Now. Drum roll here.

The number one mistake new writers make in their stories for children is

1) No real conflict - There’s no story problem. Your POV character needs to face some big problem right at the start of the story. Then, he or she needs to struggle and struggle with this problem as he/she tries to solve it. That is, things need to keep getting worse and worse until finally the POV character is able to solve the problem (or at least resolve it) and change or grow somehow in the process. Without a story problem you have what editors like to call “an incident,” and editors don’t publish incidents. They publish stories.

So that’s my list of top 10 mistakes new children’s writers make. Use this article as a checklist when you’re writing for children. Avoid these mistakes and you’ll be well on your way to publication.

Sex Drive - The Answer Is Not Always In A Pill

Is your sex drive a little low lately? Feeling a bit fatigued in the bedroom? There are ways to kick up your desire for sex and the answer is not always in a pill.

Regular exercise may be just the ticket to improve a sex drive. We all know that exercise can improve heart health, help you lose weight, and make you stronger, but research now shows that a regular exercise routine may have other benefits as well.

When we exercise, especially cardiovascular exercise, our brain releases endorphins, our “happy” neuro-chemicals. These chemicals in our body enhance mood and general feeling of well-being. For people who are stressed out or fatigued, this can mean an improved sense of clarity and energy. Why not take that into the bedroom? Feel good about life and you will inevitably feel good about sex. Feel good about sex and you’ll want more.

But the energy boost isn’t the only benefit of regular workouts. Perhaps the greatest benefit of a regular exercise routine to improve a sex drive are the physical improvements. Just in terms of how we feel, working out may make us feel differently about our bodies. Keeping your body slim, strong, flexible and injury and sickness-free is a sure way to more confidence or at the very least, an easily moving body. This overall wellness can mean a greater desire to show off and use your body.

Good sex requires strong muscles and any exercise routine that increases core strength is a great choice. Your abdominals, glutes and low back are very much involved in sex and keeping them strong and toned may assist in better orgasms simply by the virtue of where they are placed anatomically. Any of the muscles of the trunk and pelvis are important, but none more important than the pelvic floor muscles.

Perhaps better described as the “best sex” muscles, the pelvic floor muscles are a group of muscles directly under your body at the base of your torso. They are like a hammock slung under your pelvis and work to control the actions of the bladder. The pelvic floor has also been linked to better orgasms. Sex therapists use Kegel exercises to help patients get in touch with this part of their body, and this goes for both men and women. Keep the pelvic floor muscles strong just as you would any other muscle in your body.

The moral of this story is basically this: If you are feeling low and your sex drive is the poor victim of stress or depression, you may find relief with a regular exercise routine. The combination of improved strength, more stamina, increased body awareness and general feeling of well-being that comes from having a regular exercise routine, may allow you to feel more excited about sex which in turn may leave you wanting more.

This is not to say that this is the answer for everyone. There are many cases where medication is the only way.
